If one has GERD because the LES is incompetent then he can expect to get reflux esophagitis if he refluxes only acid. If he refluxes a lot of bile with the acid many experts believe this leads to the Barrett's mutational changes in the esophagus. This sometimes leads to cancer. If one has reflux with position and chokes at night then they can die from aspiration. I have had such patients who were afraid of having surgery and paid the ultimate price to avoid it.
