I need a little clarification of your pain. Where exactly do you get the pain (mid back, right side, up toward your shoulder, etc.) and does it occur primarily following meals? Generally, severe pain is not associated with just reflux disease and another cause (including musculoskeletal pain) needs to be entertained, such as gallstones, paraesophageal hernia, esophageal diseases, or something completely unrelated. Describing the site of the pain, what makes it better or worse, the timing of the pain in relation to activity, meals etc. are all important symptoms to help discern the cause. But I would not necessarily relate it to the reflux.
