I had exactly the same problem - Prilosec worked for 3 years then the heartburn returned. My thoughts were to maybe go off it for a few months every year, then go back on. My doctor told me the body does not develop a tolerance to the drug, just the initial problem (Hiatus hernia in my case gets worse. So instead he advised me to go back on the "bland" diet - where you give up all the fun foods - chocolate, spices, alcohol, etc. He said surgery for hiatus hernia is seldom a success so he does not advise to go that route.
I hope you find something else that works for you!
