Severe GERD for over 8 years

I have had GERD from 1992. I have gone thru all the tests (EGD, 24PH, manometry, Gastric Emptying) and all of them have showed that I have severe GERD. By the way to make things worse I also have a sliding hiatal hernia. I took Axid for about 7 years and I have been taking Prilosec or Protonix or Prevacid.

I am currently taking 2 Prevacids (30mg) but it is not improving my symptoms. On my bad days(which is currently everyday) I have severe chest pains, sour taste in my mouth, hoarseness, etc.

What are my options? Endo Cinch, Stretta or Nissen? My GI Doc does not believe in the above options because of the side effects or not proven yet procedures. I am 35 so How long do I need to put up with these symptoms? Should I just go in for the Nissen? Is there a good surgeon in Chicago area?
