You have so many symptoms that it seems unlikely they are all related to one particular problem. From what you wrote, I am not able to determine how severe the symptoms are. Is certainly possible that some of the problems are related to reflux disease. As a simple test, I would recommend treatment with a proton pump inhibitor to see if any of your symptoms improve. I would encourage you to continue seeing a doctor you felt comfortable with so that each of the individual issues can be dealt with systematically
