Rodster: Disruption of a Nissen Fundoplication, or a slipped wrap is a known complication of surgery. For a lot of reasons, sometimes things just don't heal in quite right. Repair can be done either through the chest or abdomen. You did not mention if the transthoracic repair that your surgeon planned was as a thoracoscopic procedure. Repairs can be attempted with a scope. I'm not aware of the national average success rate of completing these repairs either laparoscopically or thoracoscopically. We've been able to do some patients, and in others we've had to convert to an open procedure. Unless there are some unusual circumstances, you can always try first with a scope and then convert to open if needed. I hope that this helps. L.Cagle, M.D.
