In addition to the pressure, other factors necessary for the LES to function properly are:1.its total length, 2. its intraabdominal length. The fact you have Barrett's and a positive 24 hour pH indicates the valve to be dysfunctional in spite of a normal resting pressure. It has to be due to one of the above reasons. I would suspect based on your findings you are a candidate for surgery if the remainder of your health is reasonable. Alan White
