Rik, PPIs bothered me a lot too, and they actually made some of the reflux symptoms worse because of side effects. All in all, they helped me more, however.
I talked to a lot of doctors between diagnosis and surgery, including two surgeons, and several of them told me that one of the "signs" that a Nissen is the appropriate solution is your reaction to PPIs. If they help you, the surgery will help. If not, surgery is NOT a good idea.
Having been through a Nissen, I am very much believe that anyone who qualifies should go for it. I cannot imagine going a day longer than necessary without it. That said, having been through a Nissen, I know that it's a major big-time ordeal, and I would NEVER want anyone who is not the right candidate for it to go through it.
Finding a seasoned expert who's not desperate for surgery and the money from it so that you can get help in sorting through your issues is a very good move. Good luck!
