It's completely normal to be scared about what we're not familiar with, Robyn. And when you don't like the doctor, it's even more normal! Those of us who've been through endoscopies don't worry about them, however, because they aren't anything to worry about (assuming general good health otherwise, compentent doctor, etc.).
If I were in your shoes, I'd go ahead with it IF this doctor has been strongly recommended to you as a competent GI doctor. We already know he's no good with people, but if his technical and diagnostic skills are above average, I think you'll be in good hands. As you'll find, you won't even talk to him except for perhaps saying hello -- the nurses will take care of you until you're out. You can then take the results to a humane doctor for follow-up.
IF you have any reason to doubt his capability, I'd not go through with it. Hideous personality doesn't count on this one. If you met the doctor who did my pH test, you might consider me certifiable for letting him even be in the same room as I was. But he knew his business, and his business was what I needed so I put up with him.
