Alcohol abuse is only one of the possible risk factos of esophageal cancer. Adenocarcinoma of the esophagus is linked to Barrett's and acid reflux and not necessarily caused by alcohol use. And yes people die of Adenocarcinoma. There are esophageal cancer support groups on the internet and the stories can be quite an eye opener. Also note that there are other cancers of the esophagus such as squamous cell cancer that may not be associated with reflux.
