Occasionally, certain patients will complain of severe sharp chest pains associated with difficulty swallowing. Most gastroenterologist and G I surgeons will then order a 24-hour pH study and esophageal manometry. If during the manometry test, there is short segment severe high pressure contractions, we make a diagnosis of Nutcracker esophagus. The only treatment for Nutcracker esophagus is to either diminish the stimulus that causes the contractions or to perform a esophagomyotomy. Medical treatments include calcium channel blockers or other antispasmodics.
