Unfortunately, the answer is most likely yes. Even though the acid is not present, the gastric juice and, perhaps to a significantly greater degree, bile often create irritation, inflammation and changes in the esophagus. This can occur without feeling the usual symptoms of burning or pain. Some physicians are concerned that the medications may mask the pain and discomfort, but the potential for the development of Barrett's changes, etc. will not be altered. Routine endoscopic examination is needed. Surgery may not cause regression of the changes in the esophagus, but it does seem to halt the progression to more concerning findings (e.g. cancer) in most cases.
